I was on Belew's show in Prague and on the setlist there was written "Madness".

Yeah, I agree it's confusing. HAving spoken to Adrian and Tony, the members of Crimson generally don't call the songs by the same names as are on the albums.
It's also "Madness" on 'Side One', and 'A Little Madness' on 'Side Four' so I suspect (I do not have Side One) that 'A Little Madness' is a shorter version. Who knows. Not worth arguing about :)
