What is considered an encore?

Does the band have to leave for a meaningful amount of time, or just leave the stage, period? What if a taped intro to a song starts playing immediately after they leave?

I would call all of your examples worthy of an encore on setlist.fm. Also, sometimes in a smaller venue, an artist might choose to play the encore “game”, but not even leave the stage because they have nowhere else to go! I also would call this an encore – it's exactly what happened at this gig.

From the perspective of the audience, how can an encore be identified, if the artist doesn't leave the stage?

Use your best judgement. Sometimes the artist says it, sometimes it's on the actual setlist.
